APPLICATIONS FOR NEW ORDINARY MEMBERSHIP

PLEASE READ CAREFULLY BEFORE APPLYING!

THIS FORM IS ONLY FOR NEW APPLICANTS, NOT EXISITING MEMBERSHIP RENEWALS.
For exising members you will recieve a communication via email or sms around the 1 week of March with the process to pay your annual membership subscription fee, if you do not, please email members@brisbanesikhtemple.org.au

For NEW applicants, to ensure your new membership application is valid and processed without delay, please complete the following steps before filling out the membership form.

What You Must Do:

  1. Download the Queensland Statutory Declaration Form
    Use the official link provided:

  2. Print the form

  3. Complete all required details 

  4. Have the document witnessed in person by one of the approved witnesses listed below.

  5. Take a clear photo or scan the completed and witnessed document
    Save it as a clear image (JPG/PNG) or PDF ready for upload.

Approved Witnesses (Queensland Paper Statutory Declaration)

Your statutory declaration must be signed in the presence of one of the following:

  • Justice of the Peace (JP)

  • Commissioner for Declarations (Cdec)

  • Notary Public

  • A Lawyer

  • Conveyancer

  • a conveyancer, or another person authorised to administer an oath, under the law of the State, the Commonwealth or another State, or

  • a person that is prescribed by regulation (currently senior police officers).

⚠️ Important:
Your application will not be valid unless the completed and properly witnessed statutory declaration is uploaded with your submission.

Before starting the application, ensure you have:

1️⃣ Photo Identification

Upload clear images of BOTH sides of your:

  • Driver Licence (front and back)

OR If you do not have a driver licence, provide:

  • Another form of government-issued photo ID (as per application instructions)

2️⃣ Proof of Address

This must confirm your current residential address.

Examples may include:

  • Utility bill

  • Bank statement

  • Government correspondence

(Refer to application instructions if unsure what is acceptable.)

As per the Constitution, Ordinary Membership is open to:

  • Any Sikh

  • A resident of Brisbane or surrounding vicinity (as determined by the Management Committee)

  • 18 years of age or older

  • A person who does not owe allegiance to any other religion

  • A believer in:

    • One Immortal God (Ek-Ongkaar)

    • The Ten Sri Guru Sahib Ji

    • Sri Guru Granth Sahib Ji

    • The teachings and utterances of the Ten Sri Guru Sahib Ji

    • The initiation ceremony of Amrit as bestowed by Sri Guru Gobind Singh Ji

Please ensure you meet this criteria before applying.

Once you have:

✔ Completed and witnessed statutory declaration (ready to upload)
✔ Clear photo ID (front and back)
✔ Proof of address
✔ Confirmed eligibility

You may now:

  1. Fill out the membership application form

  2. Upload all required documents

  3. Pay the applicable fee

  4. Submit your application

New membership applicants must provide:

  • Completed and witnessed statutory declaration

These documents must be uploaded with the application for it to be considered valid.
